Could someone please point me in the right direction for creating a geotiff DEM with Geotools that from gridded X,Y,Z coordinates.

The integers you are working with are the row and column indexes of the underlying raster object. To find out what these values should be when dealing with geographic coordinates you can use the worldToGrid method of the GridGeometry class.
